Client/Counselor/Company Privacy Agreement

• This agreement is between the client and the counselor;
• I, the client, have read and understand the Informed Consent informed consent and give the counselor right to treat me for the mental health disorder;
• I understand that my counselor and MACLSCS Ltd. will respect my confidentially and the contents of the sessions will not be discussed with other members of the family, friends etc... unless I give a verbal/written authorization to the counselor to do so;
• If at any time the counselor feels that I am a threat to myself or others, the counselor reserves the right to notify appropriate authorities and intervene with appropriate referrals to optimize my overall well-being.

Refund Policy:

�         MACLSCS Ltd. refund procedures are handled depending on the circumstances.  A client missing or being too late to enter the session is not a valid reason for refund and the refund request will be rejected, however, we may offer to reschedule a session at no additional cost.  All other circumstances will be evaluated on a case by case basis by our Executive Board or Ethics Committee and you will be notified of the decision within 30 days of submittal.  To initiate a refund please send a message to info@myrinaconsulting.com which includes the following.

o    Consultant Name:

o    Session Type:

o    Session Date and Time (in your local time):

o    Reason for Refund: (please be specific)

Security Policy:

�         MACLSCS Ltd. puts securing our customers� personal information as our highest priority. We understand that every time a customer provides us with their credit card and other sensitive, personally identifying information, they trust that we will protect it � and this policy is designed to ensure that this trust is not misplaced.

�         Personal Information is secured not only by encrypting the data we store but we also separate the data and store it in different locations.  That data can only be drawn together using our encrypted, SSL (Secure Socket Layer) connection system and having the specific request keys for each data set. 

�         MACLSCS Ltd. utilizes a 3D-secure/Verified-by-VISA PCI compliant payment processor who is responsible for collecting and transmitting all sensitive payment information. By meeting the Payment Card Industry's rigorous data security standards (PCI DSS v1.2), your payment information is tightly secured, and only transmitted to VISA through an encrypted, SSL (Secure Socket Layer) connection. We do not collect or store any of your payment details, as this is handled by our PCI compliant payment processor. We have chosen our payment processor carefully, having reviewed their security mechanisms and practices, we know that your personal information is handled with the utmost care and security.
